All of these forms are .pdf documents, so you will require Adobe Reader or another program that displays .pdf documents. We recommend that you fill out these forms on the computer and then print the document from your computer.All documents must be fully completed and signed before they can be accepted by CRHC.

I need to complete my annual update/income review

If you are on a waiting list for a CRHC program, please download and fill out this form: Application Update

If you are in a CRHC rental program and need to complete your Annual Income Review, please use the Annual Income Review Generator to get all of the necessary forms.

I would like to get a pet

If you are part of the Community Housing Program or Affordable Housing Program, please download and fill out the Pet Application Form. Please note that if you are part of the Affordable Housing Program, there is a $150 non-refundable pet fee.

Service dogs are not considered to be pets. However, the pet application information and a pet addendum are still required. Tenants are requested to submit proof of service dog certification for their dog with the application.

If you are not part of one of these programs, any questions about pets should be directed to your landlord.
