This page is used to determine which forms you will need to fill out when applying to CRHC. Answering these questions determines the forms that you will need, and fills out some of the common information on these forms. The forms can be filled out on the computer, but every form must be fully completed, printed off and signed before you can submit it to CRHC.

Public Computer Warning

Completing fillable PDF or web site forms from a public computer such as a computer in a library or internet cafe could pose a security risk. Information that you enter may be cached (i.e., saved to the computer's hard drive) and could be exposed to subsequent users. Therefore, we recommend that you do not enter your full name, children's full names, address, social insurance number, telephone number, or any other information that can personally identify you or your family members during this process if you are using a public computer. If you must use a public computer, we recommend that you clear the computer's cache and delete any downloaded files after use. Most modern browsers feature a private browsing mode which deletes any saved information once you close the browser window. Search for Private Browsing in the browser's Help menu for more information.
